PUBLIC HEALTH NURSE

COMMUNICABLE DISEASE AND CHILDHOOD LEAD PROGRAM

 

Kenosha County Public Health is looking for a self-directed, nurturing, and adaptable individual to join our team of Public Health Nurses. The Public Health Nurse provides nursing services in a variety of settings and assumes a complex caseload; works to advance health equity; maintains and promotes health; and protects and preserves a healthy environment for the well-being of all residents in Kenosha County.

 

WORK SCHEDULE

  • Full time, 40 hours per week, generally Monday through Friday

  • May occasionally work evenings and weekends

 

DUTIES AND RESPONSIBILITIES

Public Health Nurses serve in a variety of different roles at Kenosha County Public Health including school nursing, home visitation, communicable disease control, consultation services, health education, health screenings, and immunization monitoring.  The Public Health Nurse position specific to communicable disease investigation and the childhood lead program includes the following duties and responsibilities:

  • COMMUNICABLE DISEASE

    • Investigates, monitors, and provides case management services to individuals and groups with communicable diseases, including sexually transmitted diseases.

    • Assesses health status and plans treatment regimens in coordination with medical provider.

    • Orders tests and medications and monitors client response, maintains records related to treatment plan, and ensures proper medical regimen is followed and documented.

    • Develops and maintains community and client therapeutic relationships.

    • Provides home visits to clients for education, medication administration, and assessment.

    • Refers clients to medical care and other community services.

    • Enforces communicable disease codes in accordance with state statutes and ordinances, health authority guidelines, and in consultation with the Health Officer.

  • CHILDHOOD LEAD PROGRAM

    • Conduct home visit to child’s residence or other sites where the child spends significant amounts of time.

    • Complete assessment on home and identify factors that may impact the child’s blood lead level.

    • Provide education and lists of interventions to parent or caregiver  

    • Complete assessment on child’s health and developmental status; refer family to primary care provider and/or additional resources as necessary.

    • Coordinate care with other members of the case management team, to include Environmental Health and Lead Programs.

    • Continuous evaluation required on case by case basis as care plan may change.

  • OTHER JOB DUTIES AS REQUIRED OR ASSIGNED 

    • Provides clinical screenings and services including, but not limited to, immunizations, blood pressure, specimen collection and testing, family planning services, counseling, reproductive health testing, and treatment.

    • Actively participates in pandemic response and emergency preparedness planning and operations.

    • Educates community groups on health care subjects.

    • Collects data from a variety of sources and enters into multiple software systems.

    • Participates in quality improvement opportunities to enhance the diversity, equity, and inclusion of programs and services.

    • Adheres to the nursing process and the fidelity of model program(s).

    • Supports policies, procedures, guidelines, and standards of model programs and Kenosha County.

 

QUALIFICATIONS

Education and Experience

  • Bachelor's Degree in Nursing (BSN)

  • One year of nursing experience required; two years of experience preferred

Licenses or Certifications

  • Registered Nurse license required

  • CPR certification preferred

Other Job Requirements

  • Valid driver's license and acceptable driving record

  • Access to a personal vehicle for use in the performance of the job

  • Communicate in both English and Spanish (a plus)
