Old Broadway Support Staff SLS (FT)
The Supportive Living Services program is looking for staff that is flexible, enthusiastic, friendly and well organized to provide direct care to persons with mental and/or physical disabilities. Staff must be comfortable with the implementation of service plans and other rehabilitation programs and be willing to facilitate and promote community integration. Staff must also ensure individuals’ medical health issues are addressed. This may include scheduling doctor’s appointments, providing transportation to appointment, following through on doctor’s orders and communicating all pertinent health issues to the assigned Case Manager. Staff is also required to implement behavioral goals, teaching strategies, monitoring procedures, exercise programs and other team recommended programs and procedures.
Must have own insured vehicle for this program to transport the individual throughout the community for social activities, appointments, grocery shopping. Must become med cert.
