LPN Weekend Evening Supervisor


Job Purpose

The primary purpose of your job is to plan, organize and direct/manage the nursing service of residents at St. Ann’s Home and Wegmans Transitional Care Center under the supervision of RN Nursing Supervisor.


Job Functions

Performance Outcomes

Management/ Clinical Assessment:

____ *Reviews staffing plan for his/her shift.

____ * Assesses Residents and facilitates care for condition changes.

____ *Receives shift report from previous day/evening/night supervisor.

____ *Gives supervisor's report to relieving supervisor.

____ *Makes general rounds on each unit. Evaluates all Residents on resident condition report.

____ *Remains available for professional and managerial support to each staff nurse.

____ *Evaluates and analyzes performance of all staff on nursing units.

____ *Evaluates continually the nursing services rendered on his/her units and directs specific Resident care when needed.

____ *Reports any concerns to the Lead Supervisor.

____ *Utilizes appropriate resources for inservice programs.

____ *Communicates with families.

____ *Obtains supplies from Pharmacy, Laundry and Central Supply Room for resident care as needed.

____ *Notifies Medical Provider, Nurse Manager Care Coordinator of changes in resident condition, receives and transcribes orders and initiates appropriate follow-up.

____ *Guides staff in appropriate and economical use of supplies, equipment and personnel.

____ *May be delegated to fill in as a team leader (this includes covering a long term care of rehab unit including medication pass).


Recruitment and Leadership:

____ *Completes incident reports with investigative follow-up for incidents occurring on shift, reviews plan of care and reports issues and concerns to the Lead Supervisor.

____ *Facilitates complaints and concerns brought to their attention on shift.

____ *Collaborates with RN Nursing Supervisors in time of emergency situation in implementing Emergency Management Plan.

____ *May interview applicants for nursing supervisors and make hiring recommendations.

____ *Coaches nursing personnel and interprets policies and procedures and standards of care for staff.

____ *Participates in nursing policies and practices evaluation.

____ *Assists with appraisals of all nursing personnel performance.

____ * In collaboration with RN Nursing Supervisors reviews staffing on a 24 hour basis and adjusts as necessary per acuity.

____ *Calls staff when needed to meet minimum standards for staffing.

Education/Research:

____ *Participates in conferences, in-house trainings and in-services.

____ *Participates in Leadership Development.

____ *May be a member of standing committees for the improvement of Nursing Service.

Miscellaneous:

____ *Reviews nursing reports from all units.

____ *Collaborates with RN Nursing Supervisor to assist with employee injuries from any department while on duty and takes appropriate action.

____ *Informs Director of Nursing of appropriate resident/facility concerns.

____ *Performs other duties as assigned by RN Nursing Supervisor, ADON or CNO.

____ *Participates in committees.

Working Conditions

While performing the duties of this job, the employee is occasionally exposed to extreme heat. The noise level in the work environment is usually moderate.



Qualifications

Education:

Current NYS LPN license, five or more years of long term care experience required. 4-5 years to complete A.A.S. in nursing.

Language Skills:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.


Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ration, and percent and to draw & interpret bar graphs.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.


Specific Requirements

Current/valid NYS license.



Physical & Sensory Requirements


While performing the duties of this job, the employee is regularly required to stand, walk, and talk or hear. The employee frequently is required to use hands to finger, handle, or feel and reach with hands and arms. The employee is occasionally required to sit; climb or balance; stoop, kneel, crouch, or crawl; and taste or smell. The employee must be capable of performing lifting/transfer tasks at the maximum weight limit of 51 lbs. Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus.



Safety Factors


The employee is required to ensure personal safety, monitor work environment and make necessary improvements while ensuring the safety of others to include, adhering to regulations set by OSHA, DOH, and facility regulations, to include fire protection/prevention, smoking regulations, infection control, etc. Tasks involved in this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
