Project HOME achieves its mission through a continuum of services comprised of street outreach, a range of supportive housing, and comprehensive services. We address the root causes of homelessness through neighborhood-based affordable housing, economic development, and environmental enhancement programs, as well as through providing access to employment opportunities; adult and youth education; and health care.

Job Summary This role is specific to Young Adult residents however; it also includes service coordination to our adult population. The chief responsibility of the Young Adult Resident Service Coordinator is to provide direct, supportive, individualized care in a positive and holistic way to residents, specifically young adults, moving towards independence. The Young Adult Resident Service Coordinator will support residents in the retention of housing, employment, the continued development of skills and participation in activities that will enhance their ability to become self-sufficient. The services also include keeping accurate records in an electronic record system.

Essential Duties and Responsibilities

  • Assist residents with coordination of benefits and resources
  • Along with the resident and supports, design and implement goal plans that focus on education, vocation, health (physical and mental), finances, family, legal issues, and discharge planning (for Young Adults) that encompasses short and long-term goals, action plans, and means for evaluation and revision. Weekly, review and update progress or lack of progress towards goals.  
  • Support residents in connections to BenePhilly to assist in filing for benefits or resolving any issues through DPW, SSA, and unemployment.
  • Assist residents in job search process and make referrals to Employment and Education Services.
  • Assist residents in coordination of their education goals, including GED testing, college application and financial aid.   
  • Assist residents with medical and psychiatric referrals, communication and escorting residents to appointments, as needed.
  • Support residents in maintaining a commitment to recovery, including conducting drug tests as necessary.
  • Support residents through the completion of housing re-certification process.
  • Understand, anticipate, and responto the issues, expectations, and special needs that each resident may have.
  • Work with residents individually as needed, including escorts (e.g. one to one counseling, providing an escort to appointments, interviews and activities)
  • Work with residents to assess needed resources and facilitate connection to resources in the community; monitor client’s participation in activities.
  • Maintain case records, as required in a timely and thorough manner including case notes, goal plans, Project HOME assessment and HUD assessments and additional assessments as required.   Utilize database for all documentation; to include intake/orientation/discharge of clients
  • Meet as scheduled with the following people:
  • Participate in monthly staff & resident meetings and special committees.
  • Meet weekly in supervision with Program Manager and attend team meetings.
  • Participate in trainings and continuing education opportunities.
  • Facilitate Young Adult Life Skills classes (individually and in a group setting)
  • Alternate on call with other residential staff for emergencies
  • Be flexible to meet the changing needs of the young adults we serve and be available by phone
  • On occasion be prepared to arrive on site during off schedule hours for emergencies as needed.  
  • Work with residents and supports through crises, assessing the situation and making needed plans/interventions.  Become familiar with all emergency procedures.
  • Building community for the Young Adult Resident Service Coordinator means:
  • Receiving and responding to work tasks in a courteous and timely fashion.
  • Meeting and greeting visitors, residents, other staff and co-workers in a professional and welcoming manner.
  • Intentionally maintaining open dialogue and verbal/written communication.
  • Striving to put others at ease and communicate in ways they can understand.
  • Developing and maintaining smooth and cooperative working relationships with others.
  • Encouraging open expression ideas and opinions.
  • Additionally, all staff are responsible for the following:

  • Establishing a base of trust with each resident.
  • Developing good working relationships among community and staff.
  • Continuing to learn about mental illness, addictions, and dual-diagnosis.
  • Accepting residents “where they are at” while working to assist them; ensuring best practices are utilized to create and maintain a diverse equitable and inclusive environment for all. 
  • Observing confidentiality, privacy, and dignity of each resident.
  • Becoming familiar with Emergency Procedures.

 

Minimum Qualifications

  • HSD/GED plus 4 year’s experience with populations served; or an Associate’s degree and 2 years experience; or a Bachelor’s degree in social work, psychology, human services or related degree
  • Experience working with at-risk populations, specifically youth and young adult population (LGBTQ work experience preferred), formerly homeless populations
  • Experience working with mentally ill, dually diagnosed populations
  • Excellent verbal and written communication, organizational and interpersonal skills
  • Computer literacy
  • Valid PA Driver’s License

 

Preferred Qualifications

  • Experience in brief counseling, assessment and judgment, crisis management skills, trauma informed practice and resiliency. 
  • Ability to handle physically and emotionally challenging situations.
